Alright, let me take this opportunity to recommend against installing the bearing/plunger combination that is not interference fit.

Here's how it ended up...

Now to be fair, the locktite I used was not the strongest sort. Perhaps if I'd been more disciplined with delaying the install til I got the stronger stuff, I would have had better results...but still it is a bandaid method that doesn't really address the inherent problem.

[EDIT: this below was my thought process at the time, but might not be the correct way it failed, see comments further down...]
As I was driving, I heard a slight squealing/spinning sound and I thought "yep the bearing is now spinning on the plunger, I hope it doesn't catch and seize". Next time I drove it - I had to drive it home - after quite a while a while I heard the noise change and sounded like stuff was going baaaad and thought "yep there it goes". Drove the rest of the way without using the clutch.

Opening her up, examination (thankfully) found that the failure was as predicted, and it looks like no other damage was done.

I saw Karcraft posted they have a good interference fit plunger/flat faced bearing combination so I've contacted them about getting those.

Let's be honest, none of us are super surprised it shat itself are we? :D

Wow mine is loctited .Rob said do knurling to fix it Good luck .My plunger was Karcraft The bearing not.
Not surprised

Knurling - good call, I wish I had thought of that. I was wondering how to increase the effective diameter of the plunger while maintaining concentricity. Not that I have a lathe...or knurling tool...or experience knurling things...

Mine stuffed up pretty quickly, so if yours is still going well, perhaps your loctite job is good and effective and mine is a cautionary tale to do it properly!

how can you tell that the bearing was spinning on the carrier before the bearing itself collapsed?

seems more like the bearing crapped itself...

if the bearing is constantly under load
if the clutch is too heavy (spring preload not set properly)
if the bearing isn't good quality

What is the condition of the surface the bearing is running on.
Is it running on a smooth surface.
Is the surface running true.

Some of the bearing faces on the springs have been known to have woefull runout and variable because it is not secured iin the spring housing at that. Check that as well.

Quick update to say that in my followup troubleshooting I've done a more thorough strip down and short version is, looks like travel distance measured at the slave cylinder end is now the most likely culprit; a clutch setup issue as a few people have pointed me to. I've found several small things that all combine together to create the issue (of my making, since it was me who built the thing). Some of them surprising actually.

I think the reason I've switched between initially having a dragging clutch, to instead solving that but creating throwout bearing issues is because I didn't have enough travel to do both things; to push in far enough to release the clutch and to pull back far enough to disengage the throwout bearing. I substituted one issue for the other.

I'll confirm that my solution does actually solve it, then post the pics of the multiple things that contributed to the issue.

I took the clutch return spring off mine 10,000 odd miles ago and backed the small stop bolt in a few turns, so it self adjusts like almost anything else with a thrust bearing. No more adjustment issues.
