It should be 022X2S2M09 (GAP) 28530
The prefix is before the slot in the panel and the serial number is after the slot.

I cut mine out during the conversion and had to get an ID Check here in Canberra. Essentially, my number was Y2GS7767 no gaps. But they haven't asked me to get it restamped which is strange.
